Dear Ms. Loke,

Re: 1 Provision of Anaesthetic for Patient Already Under the Care of an Anaesthetist

2 Planned Extension of Morning List Into Afternoon List

At a meeting of the Anaesthetists of Subang Jaya Medical Centre held on 4 April 2000, the above issues were discussed and the following decisions were made.

1.      Provision of Anaesthetic for Patient Already Under the Care of an Anaesthetist
If a patient is being scheduled for any procedure requiring the presence of an anaesthetist and the patient is already under the care of a a specific anaesthetist, we would like to reaffirm that the same anaesthetist should provide the anaesthetic. The most common situation is the performance of a tracheostomy on a patient in ICU or CCU and inutbated.
While we will endeavour to arrange directly with the primary physician and the surgeon concerned so that the anaesthetist concerned provides the anaesthetic, there will be instances where the surgeon books the case directly without informing the anaesthetist concerned. In such instances, we would appreciate it if you could help us out by informing the surgeon that we, the anaesthetists, have agreed that, whenever possible, the same anaesthetist should provide the anaesthetic and that he should try to either put the case into one of the lists of the anaesthetist concerned or arrange with the anaesthetist concenrned a time that is mutually convenient.

2.      Planned Extension of Morning List Into Afternoon List
When a long case booked into a morning list is expected to extend well into the afternoon list, we have agreed that another anaesthetist should be asked to take over the afternoon list of the anaesthetist doing the long case. In the event that the long case should finish early, the second anaesthetist should continue to provide the anaesthetic unless he voluntarily elects to "give back"  the list to the first anaesthetist.

As always, should you have any queries, please feel free to contact me for any clarifications.
