SCHEDULE B

1)                       Anaesthetist fees for surgical procedures shall be calculated as follows

a)                      The general charge is 35% of the fee charged by the surgeon or operating physician
                          unless a lesser amount is agreed. to with the attending surgeon.

The anaesthetist fee includes the following :

Pre-operative care, as well as care during a procedure and the immediate post operative care in the recovery room and the 1st 24 hours after the procedure if the patient returns to the ward. Beyond 24 hours, or if the patient goes to a special care unit, the anaesthetist will attend to patients in emergency situations and/or upon request of the patient's primary doctor and charge accordingly.

b)                       The surgeons fee shall be determined by the surgeon according to the attached
                           Professional Fee Schedule.

2)                       A separate fee schedule will be utilized for consultations and procedures which are not
                          performed in conjunction with a surgical procedure.
