Dear Colleague,

I have been selected as one of the committee members of the Pharmacy & Therapeutics Committee together with Dr. Robert Liew.

I have noticed that halothane seems to be very infrequently used if at all. If no one is using it at all, I would like to propose to the Committee that it be discontinued. However, if anyone is using it, even if only rarely, then I shall take no action.

Please indicate below whether you want to have halothane on the formulary or whether you do not mind if is taken off our formulary. Unfortunately, the next meeting is on 31 October 2001 and so I would appreciate it very much if you could all give a reply by 17:30 h 31 October 2001. I know it’s very short notice and I must apologise for that.
